The NJ nitrous permit is good for two years. The fee is now $25 per permit, at each renewal. They used to be free, but NJ likes to keep their hands in your pocket, so they started the fee for the permit about a year ago.
Technically, you need a permit for each place you plan to buy, because when you get the permit, you get two copies - one for you and one for the place that sells you the nitrous.
Having the permit is a good idea. Nitrous oxide is classified under the controlled dangerous substance abuse (drug) laws... that's why you need the permit. Without it, they could charge you with violation of controlled dangerous substance laws.
