it was tensioner
driving to work today the belt fell off again, this time i could clearly see the misalignment of the belt due to the tensioner being completely cracked where the pulley itself meets the spring assembly inside. Managed to hold itself together for the remainder of the ride to work and then home this afternoon.
20 bucks, 10 min, and two screws and the new on is on
