Take a strait edge and place the bottom of it on the crank pulley. Keep pressure on the strait edge from applying pressure at the crank pulley side, verify that no pulley sits in. Now slide your hand down the strait edge to the pulley you are checking and out pressure on that side. Make sure it's not the crank pulley that is recessed.
Repeat for each pulley.

Another thing that will cause a belt to get thrown off is a pulley with excessive wobble.
