that rod end was bent the jam nut is cocked at a angle, how old is it and what size rod end , scott have any off road trips lately. If not then maybe a rod end up grade is needed or a bigger size rod end. it takes a whole lot of force to bend one of those. mcmaster cataloge might event have the strength ratings for them.
