As far as i know its a one time fee, but I'm honestly not 100% sure. I'll look into it for you if no one else can answer it.

The NJ nitrous permit is good for two years. The fee is now $25 per permit, at each renewal. They used to be free, but NJ likes to keep their hands in your pocket, so they started the fee for the permit about a year ago.
Technically, you need a permit for each place you plan to buy, because when you get the permit, you get two copies - one for you and one for the place that sells you the nitrous.
Having the permit is a good idea. Nitrous oxide is classified under the controlled dangerous substance abuse (drug) laws... that's why you need the permit. Without it, they could charge you with violation of controlled dangerous substance laws.
I have the info for getting the forms if you need them.

The address for getting the form and the name of the form you need are right on the copy of the permit that Slow 88 posted - look at the top of the form. Write to them there and ask them to send you the forms. Once you get a permit, they will mail you a renewal when it is due. Last time I got mine it was still free, but I think it expires in the next 3 or 4 months.
I have had places like Shore Motorsports ask me to show them the form, but they have never kept their copy. Anybody actually have a shop ask to keep a copy of the form with their name on it?
