Nitrous Oxide Permit

I'm pretty sure this has been discussed before...

I know in NJ you need a permit to handle nitrous, can someone point me in the right direction of where I can apply for one?

NJ Dept of Health and Senior Services issues it (I think). Been a while since I had mine.
